Registration forms are available from reception. You will also be asked to fill out a simple questionnaire. You will be registering with the practice rather than an individual GP but you are free to see a GP of your choice. Once registered, you will be asked to attend an appointment with the Healthcare Assistant, Practice Nurse or GP.
